7 neck support pillows tested – 6 fail miserably. Only one is convincing.
Written by Mary Roberts
Many people with neck pain or poor sleep try out different pillows for years – and wonder why nothing gets better.
But what if the problem isn't in your back... but the pillow under your head?
The fact is:
Incorrect head posture is an underestimated sleep-killer. It's also the main reason why millions of people wake up in the morning with pain, tension and leaden fatigue – even if doctors often cannot find a clear cause.
In everyday practice, it looks like this:
The worst part:
Many people are content with exactly this state of affairs: "My pillow is just comfortable." "I've gotten used to it." "It's not that bad, really."
But the reality is:
What feels "comfortable" costs you energy, focus and joy in life every night.
Our spine follows a natural S-curve. If it becomes flattened, twisted, or overstretched while lying down, your body reacts with constant tension – meaning your neck is under tension all night.
At the same time, incorrect head posture narrows the upper airways.
A seemingly harmless pillow can therefore become a ticking time bomb: poor sleep promotes cardiovascular diseases, obesity and depression.
A good pillow is worth its weight in gold: Because it prevents all these problems by stabilizing the spine, relieving the muscles, and opening the airways.
Provided that your pillow suits your sleeping position, body shape and mattress height. Our test winner is so flexible that you don't have to worry about that anymore (more on that below).
We examined 7 of the most well-known neck support pillows – focusing on precisely the problems that many underestimate: Poor posture, restricted breathing, and unsuitable pillow height.
We wanted to find out which pillows really provide relief in everyday life – and not just impress on paper. Most models are solid, however only one of the tested pillows was able to fully meet our strict test criteria.
To ensure a fair comparison, we evaluated all pillows according to the same criteria:
Here's how the individual models fared in our test:
Billerbeck is a renowned traditional brand – but the Rondomodel shows that reputation alone is not enough. The pillow is solidly made and feels pleasantly soft (fluffy cover, point-elastic core). However, it offers hardly any real ergonomic innovation. Those switching from a classic down pillow may find this a gentle introduction – however, the support is limited for those with genuine neck pain. Many customers report that the support strength diminishes relatively quickly.
Advantages:
Proven brand quality (Billerbeck)
Comfortable lying position with high pressure relief
Easy to care for and allergy-friendly (cover washable up to 200 °F)
Disadvantages:
No special ergonomic contour (just a simple cylindrical core)
Not height-adjustable (no adaptation to individual needs)
Little added value compared to a standard pillow (support diminishes quickly)
The Sissel Classic Plus is a well-known orthopedic pillow from the rehabilitation world and is often recommended by physiotherapists. It offers targeted neck support thanks to its pronounced L-shape and integrated neck roll. Thanks to the removable Vario insert, it can even be used in the Height adjustable (4.25 inches or 5.5 inches) and can be adjusted to shoulder width and sleeping position. The polyurethane foam core actively supports the cervical spine and brings it into a healthy posture.
However, the unusual shape requires an adjustment period – the manufacturer recommends approximately 2 to 14 nights. It is unsuitable for stomach sleepers, and very broad side sleepers may find the compact pillow width (13 inches) limiting.
Advantages:
Proven orthopedic design for neck problems (hardly changed for 35 years)
Removable velour cover (washable) and durable core (5-year guarantee)
Height adjustable thanks to removable Vario element (individual adjustment)
Disadvantages:
High price without major innovations (classic concept, ~ $75)
Takes some getting used to due to its unusual shape (several nights of adjustment are necessary)
Not ideal for all sleeping positions (unsuitable for stomach sleepers; only conditionally suitable for side sleepers with very broad shoulders)
This orthopedic memory foam pillow with a bamboo cover is one of the best-selling products on Amazon – mainly thanks to its affordable price. The soft cover made of natural bamboo fibers feels very pleasant, and the viscoelastic foam filling adapts well to the head and neck.
For people with a height of approximately 0.65 inches or more, the height of about 6 inches offers optimal support. Many users report less tension. However, it has no height adjustment or special contour – it is a continuous foam core in a standard shape. For serious problems, there is a lack of ergonomic differentiation. Furthermore, the dense memory foam can lead to heat build-up, as no active ventilation channels are integrated.
Advantages:
Very good price – affordable model (~ $50)
Pleasant, skin-friendly bamboo cover
Adapts to the body with point elasticity (memory foam provides noticeable relief)
Disadvantages:
Not height adjustable (fixed height – only optimal for people >5.25 feet)
Lacks pronounced ergonomics or contour – unsuitable for severe discomfort.
Heat generation possible (possibly too warm for heavy sweaters)
A true all-rounder in the middle price range: The Casimum Ergo Vario is height-adjustable (a 0.75 inch thick insert can be removed) and has an ergonomic wave shape. The core consists of dimensionally stable memory foam (density 55) with integrated ventilation channels and a breathable "climate cover". This ensures a pleasantly dry sleeping environment.
When lying down, the pillow provides medium-firm, pressure-relieving comfort – equally suitable for side sleepers and back sleepers. The price-performance ratio is very fair. For very broad shoulders or special requirements, the standard size (23.6×11.8 inches) may be too compact – but the manufacturer also offers a larger XL model. Minor manufacturing defects such as initial odor or zipper problems have been reported on a few occasions.
Advantages:
Height adjustable via removable 0.8 inch element
Comfortable lying position, good ventilation (climate cover & channels)
Fair price in the mid-range
Disadvantages:
A relatively unknown brand
Minor manufacturing defects (e.g., Velcro, zipper, initial odor)
Standard size may be too narrow for very broad shoulders.
The Ergolex Premium Neck Pillow conveys a high-quality impression: The Tencel-polyester mix cover feels pleasant, and the viscoelastic foam core is durable and equipped with ventilation channels. The ergonomic shape (19.6×11.8 inches with head recess) ensures stable alignment of the cervical spine in side and back positions.
However, there is a lack of significant innovation. The cushion is not height-adjustable and rather compact – some testers would have preferred a larger surface area. For average sleepers, it offers a solid but not very flexible solution.
Advantages:
Good support for side and back sleepers
High-quality materials (Tencel cover, airy memory foam)
Easily available (including travel bag)
Disadvantages:
No innovations or customizable elements
Not height adjustable (fixed height of 3.9 inches)
Relatively high price for the service offered.
The Blackroll Recovery Pillow is in the premium class – and you can tell: The workmanship, choice of materials and functionality are excellent. Made in Germany, the Blackroll relies on sustainable foam production with natural oils and offers a 10-year guarantee on the core.
The ergonomic shape (19.6×11.8 inches) allows for four lying positions: flat side, curved side, neck roll, shoulder recess. This makes it versatile and suitable for side and back sleepers. The open-pored foam remains pleasantly cool and dimensionally stable.
The pillow is ideal for sporty and health-conscious users. However, an adjustment period of about 1-2 weeks is needed. The price is also in the upper segment – but in return you get a durable, versatile product.
Advantages:
Made in Germany – sustainable production, 10-year guarantee
Ergonomic 4-in-1 design for side and back sleepers
Durable & breathable (ventilated memory foam)
Disadvantages:
Very high price (~ $100)
An adjustment period is needed (1–2 weeks).
Compact format – a bit tight for very broad shoulders
The unvarnished truth: Compared to Derila, the other pillows felt like cheap placeholders. While other models score points in individual areas, Derila combines comfort, ergonomics and value for money in a way that seems almost unfair to the supposed competition. The ergonomic butterfly shape supports the neck, shoulders and head simultaneously – without unpleasant pressure or the feeling of sinking into the foam. Whether in side, back or mixed position: Derila remains dimensionally stable, provides support exactly where needed, and thus ensures noticeable relief.
The breathable memory foam also ensures a pleasantly cool feeling while lying down – even during prolonged use. The stable construction prevents your head from sinking in too deeply or your spine from bending – a common weak point in conventional neck pillows. The combination of ergonomic function, comfort and flexibility makes this model the most well-thought-out solution in the entire test field.
Special butterfly shape with supporting wings
Supports all sleeping positions
Relieves neck tension and opens airways
Can noticeably reduce snoring
Good workmanship, removable cover
Very good value for money
Availability throughout the US
Disadvantages:
Frequently sold out
Available only through their own shop
Derila also impressed in long-term testing: stable, cool and relieving – night after night. Particularly impressive: Compared to other premium pillows, Derila is significantly cheaper – even though it performs better in terms of effectiveness. Many test subjects experienced a dramatic improvement within just a few nights: less pain, significantly more restful sleep, and noticeably more energy during the day.
Ergonomic shape & support height: The butterfly design optimally distributes pressure and stabilizes the spine with pinpoint accuracy.
Material & comfort: Breathable memory foam with pleasant firmness and good climate regulation.
Customizability: No height adjustment – but the standard shape fit 9 out of 10 people perfectly in the tests.
Versatility: Ideally suited for back, side and mixed sleepers.
Price & availability: Available immediately, fair price level, fast delivery in the US.
For many, Derila marks the beginning of a completely new sleep experience: Their head is finally stable, their neck relaxes – and after years of poor sleep, some test subjects experience for the first time the feeling of being truly rested. Many testers reported changes they never would have thought possible: Pain that you would have put up with for years disappeared within a few nights. The Derila Ergo Pillow gave many people the feeling of truly deep and restorative sleep for the first time. And the best part: The effect lasted beyond the first few nights – even after several weeks, most reported consistently high levels of sleep comfort.
One participant with chronic neck pain put it this way:
"I had almost given up on getting better sleep." Now I wake up with a clear head again – without pinching in my neck, without the usual pulling sensation.”
And another test subject, whose husband had been snoring loudly for years, reported:
"My husband has been snoring for years – with the Derila pillow it has become noticeably quieter." Finally, we are both sleeping better.
This is exactly the feedback we heard again and again during the test: less tension, more restful sleep, better start to the day. Physiotherapists have also confirmed: An ergonomic pillow that keeps your head and neck in the correct position can relieve pressure on your spine and keep your airway clearer – a crucial key to restful sleep.
It's no wonder that Derila stands out so clearly in our comparison. If you want to get one, you shouldn't hesitate too long: If you don't order now you risk sleeping with pain for weeks, while others have long since started their day refreshed, pain-free and full of energy.
